The following documents should be enclosed with the note verbale/official letter: Application form for Work Permit, 2-inch photo*1 (white background), Copy of income tax certificate (does not apply to first-time applicants), Employment contract, the applicant's BTE card (if any), copy of passport, visa and ID card, the official(sponsor)'s ID card. Only reciprocal DMFO and their staff members qualify as applicants.
