The following documents should be enclosed with the note verbale/official letter:
1. Copy of the driver's license from the applicant's native country (a Chinese translation should be included unless the license is in English).
2. Two 1-inch photos (white background).
3. Copy of ID card.
4. Copy of passport.
5. Driver's License Application Form.
Only reciprocal DMFO and their staff members qualify as applicants.
